Care workers expressed fears that a mother could harm her children just weeks before she killed them - but claim their warnings were ignored by social services chiefs.

In January accountant Sakthivel Vageswaran, 36, found the bodies of his sons, five-year-old Anopan and eight-month-old Nathiban, in black bin bags when he returned from work to his home in Harrow, north-west London. Wife Jeyavani was also dead.

It has now been reported that a series of mistakes were made in the weeks leading up to the deaths.

Managers did not take calls from members of Children's Social Care at Brent Council who wanted the children taken away from Mrs Vageswaran over concerns for her mental health, it is alleged.

It is impossible for Social Services to be everywhere and solve every case. I mean do you want everything you do as a parent to monitored to the max, full investigations on the slightest suggestion that there may be something, every bruise or bump your child goes to school with have a 6 man investigation team rocking up to your house?

the system (with Skynet or not) still does not operate well when given Zero Sum statements. While Humans are Humans we will never get it to Zero.  Neutral

Yet another serious case review.
After the assessment the social worker raised significant concerns.
I cant imagine how the Managers can defend the decisions they took
