possibly a very small fraction.

I think its more to do with mis-management of funds, creating unnecessary paperwork, contracting out services that don't get completed on time and on budget and then have to have yet more money spent on it, too many managers and not enough front line staff, and also down to the users' stupidity by doing dumb stuff or alcohol related incidents etc....

plus the fact that I think any bureaucracy once it gets too big becomes way to herd to manage properly, same with any business conglomerate - they keep creating "new divisions or departments " or splitting into new companies.
